Job Description

The Care Transitions Specialist (CTS) will function as an integral part of a multidisciplinary team dedicated to providing recovery-oriented, person-centered care at Valley Cities. The CTS will work with clients transitioning from higher levels of care into lower levels of care; supporting and contributing to the care transition plans from hospital, jails, and courts. The primary responsibility of the CTS is to assist clients with engagement into mental health and/or substance use disorder services (as well as medical, social, educational, and other services) upon discharge and re-entry. The CTS will monitor and provide services for individuals enrolled in assisted outpatient services (AOSP) and on less restrictive orders (LROs) to increase engagement in outpatient services with a focus on reducing or preventing more acute illness, high-risk behaviors, re-hospitalizations and other emergency medical or crisis responses.

Essential Job Functions: (This is not the job description in its entirety)

  • Monitor and provide services for individuals enrolled in assisted outpatient services (AOSP) that support the transition of individuals conditionally released from psychiatric hospitalization into outpatient services
  • Participate in clients discharge planning and continuity of care planning in order to minimize gaps in care
  • Utilize a person-centered approach when delivering services and collaborate with the care team to support treatment goals
  • Provide or monitor the provision of court ordered services including psychiatric and medical components of community support services
  • Collaborate with referring care team, receiving care team, and care transitions team to ensure continuity of care
  • Provide ongoing coordination with Peer Bridger to support the client’s treatment goals

Minimum Qualifications:

  • Minimum of BA/BS in Behavioral Sciences field
  • Two years of clinical experience providing treatment
  • Obtain Agency Affiliated Counselor registration within the first week of hire
  • Pass required background checks.


  • Experience working with populations who are incarcerated and/or psychiatrically hospitalized
  • Knowledge about community resources and experience utilizing excellent communication and advocacy skills to assist in accessing needed resources and supports


  • Must have a valid Washington state driver’s license and reliable transportation with current insurance and the ability to transport clients
  • Computer proficiency; electronic health record experience a plus.
